Complex Care
Complex needs deserve care that goes beyond the basics.
Having complex care needs shouldn’t stand in the way of a meaningful, active life. Our Complex Care service provides skilled, person-centred support to people with complex, multiple or changing needs — in their own home, a supported living setting, or within their community. Through our Continuing Care framework across Hertfordshire and Essex, we build personalised care packages with health and social care partners, from part-time support to intensive round-the-clock care.

Complex Care
How we work
Step 01
An initial conversation to understand the person’s circumstances
Step 02
A full, person-centred assessment, including relevant care plans and professional input
Step 03
A personalised care plan, with staff trained and assessed for competence before working independently
We aim for a consistent team who build real understanding of the person over time, and we take a positive approach to risk — asking not just what could go wrong, but what someone stands to gain, and how we can support them to do it safely.
